I guess I should have read ahead and reminded myself of this question yesterday. Especially since I posted an image of Duchesne with approximate location of the medical center. I was born right in Duchesne unlike many of my siblings who were born in Heber. Duchesne had Doctor Smith and I have included a partial of my certificate so you can see my father's and mother's ages at the time.
I decided to leave some of the information blanked out because those who need this information will already know it but other possible general public will not. Hopefully they won't see this blog anyway but you never can be too careful.
